Medical Records

Medical records are not educational records and are set apart from all other Texas A&M University-Kingsville records. 

All information obtained at the Health Care Clinic is strictly confidential and is protected by professional ethics and law.  Medical records are not released to any other person or agency within or outside of TAMUK without the patient’s written consent, except as prescribed by law.  In order for the Health Care Clinic to release a patient’s medical information, the patient must complete a Release of Medical Records Form. 

Students who are enrolled for the current semester and have paid the student health fee have access to the Medical Portal and may request their records through the portal by using the following directions. 

  1. Go to the Medical Portal at
  2. Go to Downloadable forms.
  3. Go to the Release of Information Authorization Form.
  4. Download the form and the follow the directions on the portal.  Do not attempt to upload the document by using the upload button; instead attach your request to a message to the clinic nurse.  If you use the upload button, the nurse will not know you are requesting a record and you may receive an error message. 

Students who are not enrolled for the current semester or who have not paid the student health fee may request their medical records outside of the portal.  Call the Health Care Clinic at 361-593-3196 for more information.
