- Chapter 30 (Military Veteran who meets eligibility requirements set forth by the Dept. of VA) (GI-Bill)
- CHAPTER 1606/1607 (Reservist or Army National Guardsman)
- Chapter 35 (Dependents of Military Veteran who is deceased or 100% disabled - service connected disability)
- Chapter 31 (Disabled Veteran who meets eligibility requirements set forth by the Vocational Rehabilitation Program) (Voc. Rehab.)
- Hazelwood (Separate Form for Veterans and Dependents who never used exemption and previous recipients)

- VA Form 22-1990 - Initial application for VA Education Benefits or Certificate of Eligibility (Veteran who has never used benefits) or VA Form 22-1995 Change of Program (if changing benefits from one school to another.
- DD 214 Certificate of Release or Discharge from Active Duty
- Copy of Degree Plan signed by the advisor
- Pre-Authorization Form - Submitted for fall/spring and for summer I/II
- Request for Parent Letter - If attending classes at another college to be used towards the degree plan on file, a parent letter is required.

- VA Form 22-1990 Initial Application for VA Education Benefits or Certificate of Eligibility or VA Form 22-1995 Change of Program (if changing benefits from one school to another)
- DD 214 Certificate of Release or Discharge from Active Duty
- Copy of Degree Plan signed by the advisor
- Pre-Authorization Form – Submitted for fall/spring and for summer I/II
- DD Form 2384-1 (Sample) Notice of Basic Eligibility (NOBE) (obtained from Commanding Officer)
- Request for Parent Letter (If attending classes at another college to be used towards Degree Plan on file, a parent letter is required along with a copy of schedule from other school)
Chapter 35 (Dependents of Military Veteran who is deceased or 100% disabled - service connected disability)
- VA Form 22-5490 Initial Application for Survivor’s and Dependents’ Educational Assistance or Certificate of Eligibility or VA Form 22-5495 Change of Program (if changing benefits from one school to another)
- Copy of Degree Plan signed by the advisor
- Pre-Authorization Form – Submitted for fall/spring and for summer I/II
- Request for Parent Letter (If attending classes at another college to be used towards Degree Plan on file, a parent letter is required along with a copy of schedule from other school)
Chapter 31 (Disabled Veteran who meets eligibility requirements set forth by the Vocational Rehabilitation Program) (Voc. Rehab.)
- VA Form 28-1905 Authorization and Certification of Entrance or Reentrance into Rehabilitation and Certification of Status (VA 28-1905 is needed for each school - no parent letter)
- Copy of Degree Plan signed by the advisor
- Pre-Authorization – Submitted for fall/spring and for summer I/II
Hazelwood (Separate Form for Veterans and Dependents who never used exemption and previous recipients)
- Form HE-V and Instructions – Hazlewood Exemption application Packet for Veterans who have never used the exemption. Form HE-P and Instructions – Hazlewood Exemptions Application Packet Previous Exemption Recipients (Veterans and Dependents). Must be submitted each semester. Form HE-D and Instructions – Hazlewood Exemptions Application Packet for Eligible Dependents for Texas Members of the Armed Forces who have NEVER USED the exemption form.
- DD 214 Certificate of Release or Discharge from Active Duty
- Letter from VA Regional Office verifying termination of eligibility of Educational Benefits can be requested by calling VA Administration Regional Office at 1-888-442-4551
- Pre-Authorization Form Submitted first semester at A&M-San Antonio and once a year before fall semester. (Paperwork needed for each school - no parent letter)
