Wellness Incentives

In order to get a Wellness Credit on your account, you will need to complete two incentives that can be found on your MyEvive account. You and your spouse will receive up to 5 personalized options/tasks to choose from on your MyEvive accounts. These options are driven by the United States Preventative Task Force Guidelines and vary based on age and preventative screenings. You can verify your completion status for the wellness premium incentive by logging into or registering for your MyEvive account.

Please remember:

  • Complete 2 Wellness Incentives between September 1 and June 30 of every year.
  • Starting September 1st, 2022, the annual wellness exam will be required as one of your Two-Step activities for the new plan year.
  • Employees must complete 2 actions, and their spouse must complete their own 2 actions to get credit
  • 12/9 employees will see a credit of $40/month and 12-month employees will see a $30 credit/month 
    • If your spouse completes 2 of their actions, you will see their credit being applied monthly as well
  • Retirees and graduate students will automatically receive the lower premium
  • New employees receive the Wellness Credit for the 1st two years of their employment
    • This includes the employee's spouse

If you or your spouse choose not to complete 2 actions shown on MyEvive, you will not receive the monthly premium reduction.

Contact MyEvive member services if you believe you have completed an incentive, but the information is not reflected in your MyEvive account. (NOTE: remember that it can take 6 to 8 weeks from the time of your wellness exam for the claim to process and the incentive to show on your MyEvive account)

*The alternate health assessment available on Well on Target through Blue Access for Members (BAM) can also count toward your wellness credit

Registering for MyEvive:

  • Go to
  • Click on “Register”
  • Enter your ID#, which is your BCBSTX member ID on your health insurance card (your UIN)(Both employee and spouse will use the employee UIN to register)
  • Click on either “Employee/Subscriber” or “Spouse/Domestic Partner”
  • Enter your first name, last name, and date of birth
  • Click “Next”
  • Enter and confirm your email address
  • Create, enter and confirm your password
  • Enter your phone number
  • Select whether you would prefer contact by email or text
  • Check the box to accept the terms and conditions
  • Click “Next”
  • Indicate which statement best describes you
  • Click “Create My Account”

Logging into MyEvive to check your incentive status:

  • Go to and log in
  • Click on the “Check my wellness incentive” card
