Door Access Requests

The form below will send a request to have door access granted or revoked for an individual within the university with an eligible ID card. The form must be completed for an individual by the staff or faculty member that is responsible for the area that is an individual is being given access to.

Please note door access requests may never be completed or requested by students, and will result in an immediate disapproval of the request.
