Group 4 Verification

You have been selected by the U.S. Department of Education to complete a Group 4 Verification.

Some FAFSA applications are randomly selected by the U.S. Department of Education for a process called verification. Verification requires student and/or parent(s) to submit official financial information for review by the Office of Student Financial Aid before an award package can be offered. Documents may be submitted in person or by mailed to the Office of Student Financial Aid

 

This process can take up to four weeks.

Dependent Student Instructions

 

Snap Benefits Worksheet Instructions (Dependent Student)

Section A

  • Enter your current information

Section B

  • Parent should answer:
  • if they receive SNAP benefits.
  • If Yes, have them check the “Yes” box, and submit documentation that can be downloaded from the Texas Department of Human Services website www.yourtexasbenefits.com or submit copy of Benefits of Eligibility notification
  • If No, please check the “No” box.

Section C

  • You and a parent must sign form

 

Child Support Worksheet Instructions (Dependent Student)

Section A

  • Enter your current information

Section B

  • Please have your parent enter:
  • Who paid the child support
  • To whom it was paid
  • The name of the child(ren) child support was paid for
  • The annual dollar amount paid for each child

Section C

  • You and a parent must sign form

 

 

 High School Completion Worksheet Instructions

Section A

  • Enter your current information

Section B

  • Check appropriate box and submit documentation along with form. Acceptable documentation for verification of High School Completion is one of the following:
    • Copy of High School Diploma
    • Copy of Final High School Transcript showing date when diploma was awarded
    • Copy of General Education Development (GED) certificate or GED Transcript
    • Official Academic transcript indicating student successfully completed a two-year program (Associates Degree)
    • If State law requires a homeschooled student to obtain a secondary school completion credential for homeschool (other than a high school diploma or its recognized equivalent), a copy of that credential.
    • If State law does not require a homeschooled student to obtain a secondary school completion credential for homeschool (other than a high school diploma or its recognized equivalent), a transcript or the equivalent, signed by the student’s parent or guardian, that lists the secondary school courses the student completed and documents the successful completion of a secondary school education in a homeschool setting.

 

 

 

Identity/Statement of Educational Purpose Worksheet Instructions (In Person)

*This form is used when the student is able to come into the Office of Student Financial

Section A

  • Enter current information

Section B

  • Submit one of the acceptable documentation of a Government Issued ID:
    • A valid Driver’s License,
    • An identification card provided by the Texas Department of Public Safety Office
    • Valid passport. 
    • Military ID
  • Student must print his/her name on form where indicated, in the presence of a Financial Aid representative

Section C

  • Student must sign and date the form.

*This form is used when the student is unable to come in person to the Office of Student Financial Aid

Section A

  • Enter current information.

Section B

    • Valid passport. 
  • This form is to be notarized by a notary in the presence of the student.
