2012-2013 Verification Instructions

Some FAFSA applications submitted to the U.S. Department of Education are randomly chosen for a process called verification. Verification requires student and/or parent(s) to submit official financial information for review by the Office of Student Financial Aid before an Award Package can be offered.

This process can take up to four weeks. 

The process for Verification has changed from previous years.  The 1040 Tax Form will no longer be acceptable.  In its place, an IRS Transcript will need to be submitted along with copies of original W-2s received from employers.

Here are some instructions to help you complete the Institutional Verification Worksheet:

  1. TO RETRIEVE A FREE IRS TRANSCRIPT LOG ONTO: https://sa1.www4.irs.gov/irfof-tra/start.do
    • If Independent please enter :
      • Social security number of primary person on tax return filed
      • Date of Birth
      • Street address
      • Zip Code and click continue
      • Under “Type of Transcript” choose “Return Transcript”
      • For Tax Year choose the 2011 from drop down menu and click on continue
      • A message will appear “We have accepted your request for a 2011 Return Transcript. Please allow 5-10 days to receive it. The transcript will be sent to the address we have on file for you.”
    • If Dependent the above process will need to be done twice. Once for you, if you filed a tax return, and once for your parent
  2. DOWNLOAD THE 2012-2013 INSTITUTIONAL VERIFICATION WORKSHEET AT: http://www.tamuk.edu/finaid/onlineforms
    • Choose the 2012-2013 Verification Worksheet
  3. SECTION A: STUDENT INFORMATION SECTION
    • Fill out completely. Print legibly preferably in black ink.
    • If items left blank, form will be considered incomplete
  4. SECTION B: DEPENDENCY QUESTIONS
    • Check the box that applies
    • If answered Yes to any question student is considered Independent
    • If answered No to all question student is considered Dependent
  5. SECTION C: HOUSEHOLD INFORMATION
    • If Independent, student’s name must be included
      • Include anyone else in the student’s household that receives more than 50% of their support from the student
    • If Dependent, student’s and parent‘s name must be included, as well as anyone else in the household that receive more than 50% of their support from the parent. If sibling is or will be attending college at least half time (6 hours) during the 2012-2013 school-year include name of college.
  6. SECTION D: OTHER INFORMATION TO BE VERIFIED
    • Check this box if someone in the student’s (parent’s) household received benefits from the Supplemental Nutrition Assistance Program or SNAP (formerly known as food stamps) anytime during the 2010 or 2011 calendar year.
      • Form 1009 – TANF/Food Stamp Benefits Notice of Eligibility will need to be submitted
    • Check this box if someone in the student’s (parent’s) household paid child support in 2011. Required information will include:
      • Name of person paying child support
      • Name of person to whom child support was paid to
      • Name of the child for whom support was paid
      • Amount of child support paid for the year
  7. SECTION E: TAX FORMS AND INCOME INFORMATION
    • Check the appropriate box(s) for the person(s) required to file a tax return
      • If Independent, copies of 2011 IRS Transcript and original W-2s will need to be turned in with worksheet.
      • If Dependent:
        • Copies of student’s 2011 IRS Transcript and original W-2s will need to be turned in.
        • Copies of parent’s 2011 IRS Transcript and original W-2s will need to be turned in.
    • Check the appropriate box(s) for the person(s) who did not or are not required to file a return
      • If person(s) worked but did not file, turn in copy of original W-2(s)
      • If no W-2(s) were received but student/spouse or parent(s) had wages earned, fill out section under caption “Non-filers MUST list employer(s) and any income received in 2011….”
    • Additional Financial Information
      • If Independent, left column will need to be filled out. If line item does not apply enter Zeros*
      • If Dependent, left column will need to be filled out by student and right column will need to be filled out by parent(s). If line item does not apply, enter Zeros*
    • 2011 Untaxed Income
      • If Independent, left column will need to be filled out by student/spouse. If line item does not apply enter Zeros*
      • If Dependent, left column will need to be filled out by student and right column will need to be filled out by parent(s). If line item does not apply enter Zeros*
  8. SIGNATURES
    • If Independent, student is required to sign worksheet. *
      • If student’s spouse received income they are required to sign worksheet
    • If Dependent, student and a parent is required to sign worksheet. *

*Please Note: If these sections are left blank the form will be considered incomplete and will delay processing of Verification.
