Travel Reimbursement Checklist
In case of an accident involving a state vehicle:
Complete an accident report as soon as possible with the Physical Plant.
Notify your direct supervisor and provide him/her with a copy of the accident report.
Supervisors are to notify their chair or unit head and provide a copy of the accident report.
Chairs or unit heads are to notify the Dean and provide a copy of the accident report.
